Extensive geographic parenchymal APHE (B) which becomes isoenhancing on PV phase of imaging (C), compatible with post-radiation changes, LR-TR equivocal based on LI-RADS TRA v2018. At 12 months post-TARE, the treated HCC is now small and completely necrotic with no areas of APHE (D), LR-TR nonviable. There is extensive radiation fibrosis in the parenchyma.
